Although the scientific explanation is all about the inner ear and balance, I also believe that it has something to do with sensory overload. There's a lot going on in a boat.........the rocking, the splishing and sploshing of the water, the horizon moving, different smells and tastes etc. All these are bringing your senses into play.
You are having to balance, whilst at the same time focus your eyesight 'cause everything is moving, whilst at the same time listen more intently as there are lost of sounds that you only hear when fishing whilst at the same time your sense of smell is stimulated by bait, fumes, salt air etc.
I think the people who get sick are subconsciously trying to focus on all and are in sensory overload. All my kids, natural and foster, have always been sick. Once they learned to focus on the fishing, or even play rummy in the cabin when they aren't fishing, then they overcame their sickness. The only kid who remains sick everytime is my autistic boy. One of the main diagnostic criteria for autism is the experiencing of sensory overload IOW, he has a medical condition which keeps him permanently in some level of sensory overload so I suppose he'll never defeat the sickness. I have to say, he's a brave little bugger 'cause he still insists on coming out with us everytime We've tried everything with him, Kwells, Ginger, the whole range, to no avail.
